Keep up the good work. I truly share your pain. I had a triple bypass in December of 2013 because I did not listen to my doctors for many years. Can't blame it on cholesterol as my LDL is 78 - been there almost my entire life. But high blood pressure and diabetes were major contributors. At my last visit, my BP was 122 / 74 and my doc was quite pleased. However after a few surgeries and the resulting meds, my A1C went from 5.4 to 6.8.
I am working very hard on getting the A1C back down to where it was before all the surgery. Black coffee is all I now drink rather than a fancy coffee. No bread, no potatoes, no rice, no pasta. Just baked, broiled or grilled meat - mostly chicken - is now what my diet consists of.
While it is not easy, especially for someone that likes food, it can be done.
Also remember this, exercise will knock down your A1C. I'm doing 30 minutes each day on my stationary bike.
